Several countries have their own unique medicinal or remedial practices that are deeply ingrained in their culture and history. It examines the significance of therapeutic herbs in various cultures – from ancient Egypt and China to Indigenous American and African traditions – from a historical, cultural, and scientific perspective. The research highlights how herbs have been utilized to treat diseases, enhance wellbeing, and sustain health by examining ancient medicinal systems, such as Ayurveda, ancient Chinese Medicine, and Indigenous healing traditions. Although herbs are natural, they do contain active ingredients such as terpenoids, alkaloids, and flavonoids. These substances can significantly alter drug metabolism, affecting the speed and effectiveness of medications. The major chemical compounds found in the herbs may alter the pharmacokinetics and pharmacodynamics of the related drugs. It focuses on the primary mechanisms of these interactions, including the effects on enzymes, primarily cytochrome P450 inhibition or induction, as well as interference with transporter proteins involved in drug distribution, metabolism, and excretion. Due to this new tactic, analytical merits are essential for exploring these interactions, and designing LC-MS/MS, HPLC, and NMR spectrometry is important for better understanding. These methods are necessary for the effective identification of different herbal components and their potential interactions with drug actions. However, there is still an extensive research deficit, especially regarding the use of multiple herbs in a given formulation and the variation in the composition of the active compounds. This chapter fills these gaps with the current data and emphasizes the need for a more thorough analysis and the development of prognosis models. All such advancements are crucial for precisely predicting interactions and formulating procedures for safely using herbal supplements with pharmaceutical products. Its primary goal is to enhance the safety and effectiveness of combined treatments, thereby improving clinical practice and advancing individualized medicine. From this chapter, we should be able to understand the chemical processes involved in most herbal drug interactions and their effect on patients' health.

India has a rich culture of judicious identification of herbs/natural products with paramount medicinal impact for effective treatment of health issues. But, with the evolution of the pharmaceutical industry, our society is tilted towards synthetic drugs with the perception of faster effectiveness and instant relief. However, these synthetic medicines offer significant side effects with lifelong suffering. Nowadays, there is an increasing interest in reassessing natural products/herbs for various health issues. Among various categories of health issues, Metabolic disorders, otherwise called metabolic problems, emerged as one of the threats to mankind. These diseases arise from irregularities in the body's digestion. Among all, the predominant metabolic issues are diabetes mellitus, obesity, dyslipidaemia, osteoporosis, and metabolic disorders. Natural products encompass substances derived from living organisms, including plants, animals, and microorganisms. Due to their low/no side effects and easy accessibility, natural products have gained attention as alternatives to conventional chemical therapies. The activities of natural products in metabolic cycles interact with specific catalysts, receptors, or signaling pathways associated with regulating metabolism. Moreover, natural products can also impact gut microbiota, which has implications for metabolic health, and can regulate appetite and satiety, thereby affecting food intake and energy balance. We aim to cater to the specificity of natural products' mechanistic action towards metabolic disease, its advancements, and possible future scope. 

With the growing popularity of herbal supplements used alongside conventional medications, herb-drug interactions have become a significant concern in contemporary healthcare. This chapter explores the pharmacokinetic and pharmacodynamic mechanisms by which herbal products can alter drug efficacy and safety. It provides an in-depth analysis of interactions involving commonly prescribed drug classes, such as anticoagulants, antihypertensives, and antidepressants, and widely used medicinal herbs like Echinacea, Ginkgo biloba, and St. John’s Wort. Herein, the therapeutic implications of HDIs are reviewed, with a focus on their causes, prevalence, and the importance of close observation. The absence of regulated dosage, the unpredictability of herbal product quality, and the lack of knowledge among practitioners and patients about potential interactions are major obstacles. Frequently documented interactions entail modifications to drug metabolism facilitated by transport proteins and cytochrome P450 enzymes, which can result in elevated toxicity or diminished therapeutic benefits. The uptake of statins can also be impacted by the inhibition of OATPs by medications such as Ginkgo biloba and ginseng. The content of the herb, the dosage, and patient-specific variables, including genetic variations in transporter genes, all affect these interactions. To predict and manage HDIs effectively, it is crucial to understand these pathways, underscoring the need for further research and the integration of clinical practice methods. This chapter emphasizes the transport method, herb-drug interactions, and therapeutic uses of these substances.

 In today's world, millions of individuals incorporate herbs into their diets or use them for medicinal purposes, alongside prescription and over-the-counter medications. Although these herbs are often viewed as natural and safe, they can interact with other drugs, potentially causing harmful side effects and reducing the effectiveness of ongoing treatments. These interactions can be either pharmacokinetic or pharmacodynamic, or both, with most herb-drug interactions occurring due to the induction or inhibition of metabolic pathways. That is why recognizing that preparations from herbal sources contain various active phytochemicals in different amounts is essential, as these can alter transporters, enzymatic systems, and physiological processes. Potential interactions between herbs and drugs are diverse and clinically significant. For instance, combining warfarin with herbs such as Ginkgo biloba, dong quai (Angelica sinensis), garlic (Allium sativum), or danshen (Salvia miltiorrhiza) may increase the risk of bleeding. St. John's wort (Hypericum perforatum) has been associated with mild serotonin syndrome when used alongside Selective Serotonin Reuptake Inhibitors (SSRIs), and it can also reduce the absorption of drugs like cyclosporin, digoxin, phenprocoumon, and theophylline. Panax ginseng, when taken with antidepressants, may induce mania in patients with depression. The concurrent use of betel nut (Areca catechu) and neuroleptic drugs has been reported to exacerbate extrapyramidal symptoms. Additionally, yohimbine (Pausinystalia yohimbe) may increase the risk of hypertension when combined with tricyclic antidepressants.

The use of licorice (Glycyrrhiza glabra) can enhance the effects of corticosteroids, while Xiao Chai Hu Tang (Sho-saiko-to), a traditional Chinese herbal remedy, has been shown to decrease blood levels of prednisolone. Furthermore, the Ayurvedic syrup Shankhapushpi may reduce plasma levels of phenytoin. These examples underscore the importance of monitoring herb–drug interactions due to the potential for altered pharmacokinetics and adverse clinical outcomes. This chapter aims to examine mechanisms derived from previous databases and clinical implementations of examining interactions between herbs and drugs through an analysis of existing clinical and preclinical research, highlighting the importance of awareness as the applications of herbal medicines and over-the-counter drugs continue to grow. Herb-drug interaction databases are structured tools or data libraries detailing the interactions between modern medicine and herbs. Natural Language Processing, like artificial intelligence tools, is used to identify pharmaceutical products and herbs automatically. To determine the polypharmacological mechanism of action of single herbs, network pharmacology studies are carried out.

The surge in interest in phytochemicals, traditional herbal supplements, and medicines has been accompanied by an increase in concerns regarding their possible interactions with conventional medications. It is necessary to establish reliable databases that are essential tools for both consumers and healthcare professionals to reduce the risks associated with Herb-Drug Interactions (HDIs). Herbal products are often considered harmless due to their natural origin; however, there is a lack of awareness regarding potential risks if they are used together with pharmaceutical drugs. Interactions between herbs and drugs may result in adverse effects, reduced therapeutic efficacy, or enhanced toxicity, posing a substantial concern for both health care professionals and patients. This chapter examines various active databases related to herbs and drug molecules available worldwide. It examines the current state of these databases and evaluates their scope, accuracy, and usability. It also explores the difficulties in creating such databases, including the intricacy of herbal formulations, the variety of potential interactions, and the limitations of existing research methodologies. This chapter also examines the emerging trends in HDI database development, including the integration of artificial intelligence and machine learning for more precise prediction and risk assessment. Additionally, it thoroughly examined the status of HDI databases to equip healthcare professionals and patients. This chapter emphasizes the importance of integrating knowledge from diverse fields, such as pharmacology, toxicology, and bioinformatics, to mitigate the risks associated with herb-drug interactions. This necessitates enhanced regulatory oversight, improved reporting systems, and increased awareness among healthcare providers and the public to safeguard patient health in an increasingly complex therapeutic landscape.

Predicting these interactions is challenging due to the absence of regulation and standardisation of herbal products and the lack of research on HDIs, necessitating more targeted research. Bioinformatics has emerged as a powerful tool to understand, predict, and mitigate HDIs by using computational approaches. By using computational methods to analyse extensive biological datasets for the purpose of researching and forecasting Herb-Drug Interactions (HDIs), bioinformatics plays a crucial role in furthering this field of inquiry. Furthermore, the establishment of databases specifically tailored to the Human Development Index (HDI) and the formulation of predictive models are examined, offering significant resources for academic researchers and healthcare practitioners. This article reviews recent advances in bioinformatics applications to HDIs, including databases and algorithms that provide insights into how computational methods can advance our understanding of HDIs and improve patient safety.

Herb-Drug Interactions (HDIs) are a growing concern in clinical practice, especially among vulnerable populations, such as children, the elderly, and pregnant women. These groups have unique physiological characteristics that significantly influence the pharmacokinetics and pharmacodynamics of both herbal and conventional medications, increasing the risk of adverse interactions. In paediatric care, St. John’s Wort (Hypericum perforatum) is sometimes used to manage childhood depression. However, it is a strong inducer of the CYP3A4 enzyme, which can reduce the effectiveness of medications like anticonvulsants and immunosuppressants. In elderly patients, age-related changes, such as reduced liver and kidney function and the common issue of polypharmacy, complicate treatment. For example, Ginkgo biloba, used to enhance cognitive function, may interact with anticoagulants like warfarin, raising the risk of bleeding. Garlic, if used under supervision, may enhance the blood pressure-lowering effects of antihypertensive medications, requiring close monitoring. Pregnancy adds another layer of complexity, as the safety of both the mother and foetus must be considered. Some commonly used herbs, such as chamomile and ginger, may interact with prescription drugs. Ginger’s antiplatelet effects, for instance, could amplify the action of anticoagulants and increase the risk of bleeding during delivery. Recent pharmacokinetic studies havelinked many of these interactions to changes in the Absorption, Distribution, Metabolism, and Excretion (ADME) of drugs. Healthcare professionals must maintain a high index of suspicion for HDIs in special populations and educate patients on the safe use of herbal supplements alongside conventional treatments.
