Intra-Cranial Arterial Stenosis (ICAS) represents a source of risk to develop
stroke. There is some genetic and ethnic determinant in having a major risk. Non
invasive diagnosis rely on Trans-Cranial Doppler (TCD) and Magnetic Resonance
Angiography, while Computed Tomography has the best accuracy to be compared to
the more invasive Digital Subtraction Angiography. The endovascular and medical
therapy options are compared, passing through WASID and SAMMPRIS trials.
